Diabetes Blood Sugar Tracker

Please complete this chart if asked to do so by the practice nurse or GP. You should record your blood sugar readings regularly for a week. This will help us ensure your diabetic control is as good is it can be – which is essential for your health.

Try to keep blood sugar levels between 4–7mmol/l before meals and less than 9.0mmol/l (Type 1) or 8.5mmol/l (Type 2) after meals, most of the time. If you have readings less than 4mmol/l, these are too low and is called a hypo. Please manage urgently according to advice and contact the surgery to discuss this if they are occurring frequently.
